Your Donation Qualifies for the

Arizona Charitable Tax Credit

Providing free healthcare to communities with limited access to resources


Your Donation’s Impact:

  • $841 provides a family of four with yearly checkups with a one-month supply of common vitamins to support general health
  • $421 provides a family of four with immune support for fighting off the common cold and flu
  • $200 provides a patient with a one-month supply of adrenal support supplements to manage stress and improve quality of sleep

About Sage Foundation for Health


Sage Foundation for Health provides funding to community clinics throughout Maricopa County which are staffed with licensed faculty physicians and third- and fourth-year medical students from Sonoran University of Health Sciences. Patients include families in low-income school districts, survivors of domestic violence, women and children recovering from homelessness and trauma, individuals living with HIV/AIDS, and patients recovering from substance use. Clinical training students treat chronic and acute health conditions by working with the patients to restore health and improve self-care choices through daily diet, exercise, and stress reduction treatment plans. These shifts give students a unique opportunity to provide treatment and gain skills and experience in a community healthcare setting and enhance their cultural competence working with a diverse patient population. Treatments often include clinical nutrition, botanical medicine, physical medicine, acupuncture, homeopathy, and prescription medicines when necessary. Thanks to generous community support, services and treatments are provided to patients at no cost.

How to Claim Your Charitable Tax Credit


Sage Foundation for Health is a Qualified Charitable Organization (QCO), eligible for the Arizona Charitable Tax Credit.  Any Arizona taxpayer is eligible to claim this credit and it does not conflict with other Arizona tax credits. This means that you can donate to each type of organization and claim each credit. If you itemize your federal taxes, your gift to Sage Foundation will also qualify as a federal deduction.

All charitable tax credit organizations have been assigned a Qualifying Charitable Organization (QCO) Code. Donors will be required to use this code when taking the tax credit on their income tax returns. Sage Foundation for Health’s QCO Code is 20723.

At the beginning of each calendar year, we will send you a summary of your donations from the previous year as well as AZ Form 321. When filing your State taxes, you will be asked if you made any contributions to qualified charities (see example image below). You’ll select the appropriate credit(s) and click continue.

You’ll enter Sage Foundation for Health as the charity name and include our QCO Code: 20723. You’ll also include your total donation amount.

After clicking continue, you’ll see the amount of what you owe to the State decrease, or the amount of your State return increase.
